Kids Center is located in the Sam Swope Kosair for Kids Centre. It is a large, brick Tudor-style building. Parking is to the rear of the building. Our entrance is on the east side of the building near our playground. Look for our name on the green awning. Please check in at the front desk, and someone will escort you to clinic.
While in our clinic your child will be assessed by a physical therapist, an occupational therapist, and a speech-language pathologist. Following the therapy assessments, your child will be seen by the neurologist, neurosurgeon, physiatrist, and orthopedist.
Prior to being seen by the many professionals in our clinic, it is your responsibility to obtain the
necessary referral/authorization for the appointment if required by your insurance company.
Please bring all insurance cards.
Insurance requires a prescription from your doctor for the therapy assessments. This can be obtained from the referring physician or pediatrician. It should read “PT/OT/ST Assessments” and include a diagnosis. All prescriptions should be sent to the Kids Center fax at 502-635-1147.
For any insurance requiring a “referral” from your primary care physician such as with Tricare/HealthNet, you should request 3 referrals – one for Dr. Laura Jacks (Orthopedist) at the Kids Center, one for Dr. Christopher Barton with U of L Physicians Neurology, and one for Dr. Catherine Schuster with U of L Physicians Physical Medicine & Rehabilitation. All referrals should be sent to the Kids Center fax at 502-635-1147.
If your child is receiving any therapies (PT, OT, Speech) outside of school, insurance may not cover
the costs of the assessments during clinic. It is your responsibility to contact your insurance
regarding coverage and if you need any further assistance with coverage please contact Lauren Smith here at Kids Center at 502-584-9781 (ext.155).
